Position Summary: Incumbent is responsible for high level screening, assessment and planning for an identified high risk group of patients primarily in the Interventional Radiology Department, but in other service areas as needed. Collaborates with the attending and resident staff, as well as nursing, ancillary, professional, and technical staff, to coordinate the delivery of high quality, individualized, cost-effective patient care. Utilizes UR criteria (Interqual) to appropriately screen patients for admission or discharge. Concurrently, 1.) intervenes to enhance patient quality outcomes, 2.) ensures clinical information in the medical record is clear and complete reflecting the patient's accurate illness , and 3.) works collaboratively with physicians to improve the overall quality of the clinical documentation and to establish accurate assignment of patient status orders (e.g., observation, inpatient). Maintains ongoing communication with the patient/family , medical staff, caregivers, and third party payers to set priorities, plan, organize and implement care that is goal directed toward quality outcomes.
Rate Minimum $106,070.40 Maximum $126,758.00
Required Qualifications: Ability to establish and maintain effective working relationships across the Health SystemAbility to maintain a work pace appropriate to the workloadExcellent written and verbal communication skills in EnglishFamiliarity of Joint Commission, CMS, CDPH and CCS requirementsFamiliarity with Utilization Review criteriaMinimum 2 years of clinical experience working on a specialty unit (Med Surg, any ICU, ED, Mental Health)Minimum 5 years of clinical experienceMust demonstrate customer service skills appropriate to the jobMust possess complete understanding of acuity levels for specific patient populationMust possess the skill, knowledge and ability essential to the successful performance of assigned dutiesRegistered NurseAll external candidates must have a Bachelor of Science in Nursing
