This position includes ancillary coordination of care across the various RAHS health centers and with our ancillary partner providers at Kellogg Eye Center and Mott Children’s Health Center, as well as close collaboration with clinical and administrative staff. The Care Coordinator will provide support for patients and families as requested by families and clinical staff. The person in this position will also provide case management of identified patients with ancillary follow-up needs.
Coordinate tangible referral needs, entering tangible resource referrals into the registry tracking system.
Coordinate daily operations of vision events and services.
Coordinate daily operations of dental events and services.
Contact families to provide tangible resources, and following up with families to determine use of resources.
Communicate with community agencies and dental and vision partners to garner resources.
Attend standing ancillary utilizations meetings.
Case manages patients with identified with vision and dental needs via the Episode of Care tracking system (EOC). Enter case management data into the EOC.
Work with RAHS clinical staff and clinical partners to ensure individual patient outcomes
Refer to primary dental and vision home providers within and outside of UMHS for establishment of ongoing care.
Coordinate medical appointments for patients as needed.
Maintenance of: daily, monthly, quarterly, annual and narrative data for agency.

Bachelor’s Degree or an equivalent combination of education and experience
Must have excellent interpersonal and communication skills as well as demonstrated success as a member of a multidisciplinary team.
Ability to work both collaboratively and independently is critical.
Must be a proficient user of Microsoft Word and Excel.
Understanding of and ability to navigate complex systems and community resources. Must possess critical thinking, assessment and problem solving skills.
Master’s degree preferred in nursing, health education, public health, social work with macro focus, or related health field.
Experience in program and staff management desired.
Strong background in the delivery of primary health care and patient centered medical home requirements desired.
Under Michigan law, a criminal history check including fingerprinting is required as a condition of transfer or employment for this position.
